• Includes take home, custom made whitening trays and professional dental grade gels.

• Complimentary consultation is included.

• Offer covers home application whitening (119), individually made medication trays (926) and provision of medication (927), unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the Practitioner.

• Offer is subject to appointment availability of participating practitioners.

• Patients will require a clean and/or radiographs within the last 6 months prior to the teeth whitening which will be at the patient's expense.

• Cannot be used in conjunction with any other offer, subsidy, Government scheme and cannot be substituted or redeemable for cash.

• All health funds accepted, please refer to your health fund for their terms and conditions. Patients are encouraged to check their health fund cover prior to making an appointment.

• Not everyone is suitable for teeth whitening, your suitability will be determined by the clinician during your free consultation.

• Services will only be provided where clinically necessary and appropriate for eligible patients.
